Chronic Disease Prevention and Management

Here are some resources related to chronic disease. I have attempted to highlight information from BC and specifically rural vs urban.

The BC Center for Disease Control has a summary page for chronic disease which breaks down data for the province. They also have a chronic disease dashboard where you can view occurrences of chronic disease in the province and filter by type, geography, age, trends, etc.

A statistics Canada article from 2019 that examines ‘avoidable’ and ‘treatable’ mortality rates (which includes chronic disease) in terms of geography .

And finally an article looking at chronic disease self-management programs in rural communities.

Jaglal, S.B., Guilcher, S.J., Hawker, G. et al. Impact of a chronic disease self-management program on health care utilization in rural communities: a retrospective cohort study using linked administrative data. BMC Health Serv Res 14, 198 (2014). https://doi.org/10.1186/1472-6963-14-198
